Phoenix Independent Midwives comprises the services of Carole Goddard, Independent Midwife, KG Hypnobirthing teacher, International Board-Certified Lactation Consultant and tongue tie practitioner, is registered with the Information Commissioners Office (ICO) under registration number Z2587598 and in accordance with the General Data Protection Regulations (GDPR) is committed to protecting your privacy. This policy provides further information about the data that is collected, what is done with it and how it is stored.
Additionally, as a member of the Nursing and Midwifery Council (NMC) working as a registered Midwife I am required to maintain professional standards in all areas of my practice which includes record keeping and the safe storage of my documents. - What information is being collected?
- In caring for you in pregnancy, labour & birth, postnatally and in discussion with families about their feeding issues I am privileged to hear details about you and your baby’s health, birthing and feeding experiences.
- I also receive personal information about you and your baby including but not limited to, date of birth, address, phone numbers, e-mail address, NHS numbers, GP details etc.
- When providing any services, I request that clients complete a comprehensive health questionnaire and consent form before the first appointment. This is updated as needed during your care period.
- I will ask for feedback on my services which can be provided anonymously.
- What if clients notice there are inaccuracies in the documentation?
- If you notice there are any inaccuracies in my documentation please contact me at the earliest opportunity at [email protected] so that I can rectify the detail and send you the updated records.
- Can parents request a copy of the details stored?
- Clients who are receiving Midwifery care will routinely receive a copy of their Midwifery care notes. Those receiving breastfeeding and tongue tie services can request to see any detail that I hold about you and your baby. I will routinely document in your baby’s ‘Personal Child Health Record’ (red book) so please make this available to me during your appointment.
- How is the information collected?
- Paper Midwifery care notes will be created for those clients I am providing these services to.
- In discussion with families about their feeding issues I will write in your baby’s red book or if not available during the consultation I will provide a separate page for you to insert into the book.

- Why is information being collected?
- For my professional requirements as a member of the NMC working as a registered Midwife in accordance with the NMC Code.
- For ongoing continuity of care.
- In support of our discussions.
- To aid recall for you as the parents and me as your caregiver.
- How will the information be used?
- Your information will be used in the ongoing care of your current pregnancy and/or baby’s current and future care.
- Your information will not be shared with anyone without your knowledge or sold on to any other businesses.
- The exception to this will be if I have any concerns about the safety of your baby or children. Under these circumstances I would be professionally and legally obligated to share relevant information with appropriate agencies without your consent.
- Each year I collage statistics on the number of clients, visits, birth, type of feeding situations, comparison of age, gender of baby and outcomes of birth and feeding. The information will be lifted from your notes but collated anonymously for audit purposes.
- How will the information be stored?
- I have put in place measures that I believe to be appropriate to enable me to safely store your data securely without risk of unauthorised access or alteration of information.
- Hand written notes will be stored in a locked archive storage facility.
- Electronic notes are kept through a web-based system – ‘Cliniko’
- All records will be kept for 25 years as legally required by The Congenital Disabilities (Civil Liberty) Act 1976.
- My devices are all protected by passwords and in some cases face recognition.
- In the event of a data breach I will notify you and the ICO.
- Who will the information be shared with?
- Your Midwifery care information will be shared with the other Independent Midwife involved in your care.
- As parents you will retain your baby’s red book and can share it will any other professionals involved in their care.
- In respect of the confidentiality of our conversations please do not share direct excerpts of your notes on social media without first discussing this with me.
- If I have any concerns about the safety of your baby or children I would be professionally and legally obligated to share relevant information with appropriate agencies without your consent.
- The consent form gives me permission to
- Liaise with other health professionals as needed.
- If I have gained permission from you to take a picture or video of your and/or your baby this will be for training and professional use only and may appear on my website, in training presentations or on my resources with your explicit consent only.
- Your feedback will appear in the feedback section on my website.
- You may request to have these withdrawn if you feel they are no longer appropriate.
- What will be the effect of this on this on the individuals concerned?
- My aim is that rather than cause any concern, my documentation will provide continuity and transparency of care for you as parents.
